Ok, the right speaker problem was driving me insane too, but I found a solution on my system. In Windows XP, go to the control panel then 'Sounds and Audio Devices.' My audio driver is Realtek, so I'm not sure if these options will be the same for other drivers, but I half-suspect this is partially a Realtek issue anyway. Click 'Advanced' under the speaker settings area in the 'Volume' tab. Then go through the list under 'Speaker setup' and select either 5.1 surround sound speakers or one of the 7.1 surround sound options. 5.1 seems to work best for me - I find the sound of footsteps to be obtrusive in 7.1 but see what works for you. That should get the sound to balance out on stereo speakers/headphones either way.
Hope this helps someone. So far I'm really pleased with the game despite its bugs and oversights like not being able to unequip your weapon. I'm pretty sure this is a hardware problem, but because you seem so godly, I'll just ask you:P. I search 'sound' and click on change sound card settings. After doing that, I discovered that my Speakers were working but my Realtek digital output is not plugged in. I haven't fixed the sound issue yet and I've spent AGES trying before I finally came full circle and ended up back at this post. I was just wondering if the realtek not being plugged in is the issue or not.
And there is no other way to change the sound to 5.1 in the Speakers tab.
